Primary Duties and Responsibilities
As a Nurse Practitioner (Family Health), you would provide primary health care, working in collaboration with a licensed physician or physicians in community-based settings for developmentally disabled adult individuals of different ages and needs. Duties would include, but not be limited to:
o Assess and manage the medical care of these individuals with developmental disabilities. Maintain usual documentation, electronic health record, and telehealth expectations. Work collaboratively with team to integrate care with specialty providers and other clinicians. Participate in On-Call telephone coverage for off hours with team. o Participate equally in caring for our medically intensive developmentally disabled Adirondack Clinic (Article 16) patients (250-300 patients, with severe issues, multiple medications, tube feeding, have indwelling catheters and are medically frail) o Home visits to individuals based on need and circumstance o Assess the physical and psychosocial status of individuals with developmental disabilities in all age groups by means of interview or telemedicine, nurse report, health history, physical examination, and diagnostic tests o Diagnose illnesses, or assess acute needs, and facilitate therapeutic and corrective measures by way of urgent or emergent evaluations, medications, therapy disciplines, specialty medical consultations, or medical interventions o Work in concert with residential nurses, clinic nurses, speech therapists, psych providers, dental, dieticians, OT and PT and direct care staff to provide services. o Participate in on call rotation with other providers. o Provide telehealth consults as needed per current telehealth guidelines. o Provide acute and chronic disease management. o Provide medication management. o Direct and order specialist consultation or referral as indicated. o Will cover needs in the greater capital region. Salary Range:
From $105472 to $117448 Annually
Minimum Qualification
MINIMUM
Qualifications:
1. Permanent service as a Nurse Practitioner (Family Health). OR 2. Eligible to 70.1 transfer to a Nurse Practitioner (Family Health). OPEN COMPETETIVE MINIMUM
Qualifications:
Must have a New York State license to practice as a registered professional nurse; and certification by the New York State Education Department in a specialty area to practice as a Nurse Practitioner. You must meet the current requirements as prescribed by the New York State Board of Nursing for prescribing medication. Specialty area must coincide with the scope of practice performed by an incumbent; this denotes appropriate parenthetic.Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
